Service Inventory Form

CPWA is completing a Lead Service Line inventory as required by the U.S. Environmental Protection Agency and New York State Department of Health. Your participation helps CPWA identify the material of the water service line serving your property.

Your participation counts. Information submitted through this survey will be used by CPWA for the service-line inventory and related funding applications. CPWA states that submitted information will not be shared or sold.

Have your phone ready

You will need to take photos of your service line and attach them to the survey. A mobile phone or tablet is the easiest way to complete the form.
